Our Geriatric Case Managers will assist you with the planning for your elder care management needs so that you and your aging parent or loved one receive the quality health care that they deserve. We provide direct case management to clients in Southeastern Wisconsin. (Milwaukee, Racine, Kenosha, Ozaukee, Waukesha and Washington Counties).
All clients receive a person-centered care plan that focuses on their individual outcomes a care manager is assigned to personally oversee the coordination of your loved ones health and long-term care needs. Our care mangers serve as advocates and assist families and clients with decisions and options to promote the health and happiness of the client. They also assist clients that live in a variety of settings including in their homes, at assisted living facilities or in skilled nursing facilities. Our care managers are experts and have extensive knowledge about available resources, costs and the quality of services available in the community. The advantages of seeking this service is to have experienced and knowledgeable case managers coordinate services so family members can relax and enjoy the time they have to spend with their loved one.
