MyChoice logo w-tagline RGBAs individuals grow older, their needs and lifestyles change. Case Management is a collaborative approach to assessment, planning and coordination of services designed to meet the social and health needs of aging and disabled adults.

My Choice Family Care operates the Family Care Managed Care Organization (MCO) through a contract with the State of Wisconsin Department of Health Services (DHS). They provide managed long-term care services to frail elders (age 60 and older) and people age 18–59 who have physical, intellectual and/or developmental disabilities.

ANS is a contracted Care Management Unit (CMU) for My Choice Family Care. ANS provides Case Management for more than 800 frail elderly and cognitively or physically disabled adults. ANS' Case Management team, which consists of an RN and a Care Manager, can help you locate the most appropriate resources and services to best meet your needs in order to remain living in your home and ensure you are able to lead the most independent life possible.
